Before you begin to complete your emergency plan, you will need to have the following details available for inclusion when requested on the form.

  1. Medical contact details: Your doctor and any other medical contact details.
  2. School, daycare or daycare details: contact details for your children’s schools, daycare or daycare.
  3. Workplace details: contact details for each family member’s workplace.
  4. Utility company details: Utility company contact details (water, gas electricity).
  5. Local Council details: contact details for your local council.
  6. Safe meeting point: identify your family’s safe meeting point, in case you get separated.
  7. Emergency contacts: identify emergency contacts – one location and one outside the city.
  8. Emergency and evacuation kits: identify where your emergency and evacuation will be stored.
  9. Vehicles: marks and registration details of all vehicles.
  10. Utility locations: identify the location of your electricity, gas and water supply in the home and learn how to disable them.
  11. Special assistance: details of all family members who need special assistance.
  12. Contact details: cell phone numbers and email addresses of all family members.
  13. Pets: All animal details, including the veterinarian’s name, contact details and medications, if applicable.
  14. Neighbors: contact details for your neighbors.
